It should include the patient’s name, postal address, city, state, zip code, area code/phone number, date of birth, gender, name of treating physician, name of the responsible party and the relationship with the patient, the identification or policy number and the group number.
Attach a copy of the patient’s insurance card to the form while QuestDiagnostics/Bill process. If the necessary information is not provided, the customer may be charged for the payment. Contact your account manager for a list of insurers that the Nichols Institute subscribes to.

The Nichols Institute engages in many government health plans. As with the official Medicare, most Medicaid programs do not permit physicians to submit requests for assistance that they do not perform.
If the patient is joined with an additional insurer, such as Medicare or a private executive, Medicaid cannot be charged until those parts are billed. Be sure to include all necessary billing information on the claim form, including the original signature of the treating physician and the Medicaid provider identification number.
In cases where the patient has been enrolled in a Medicaid HMO program, please make sure the QuestDiagnostics/Bill is a participating provider in that particular program before submitting specimens for testing.
Medicaid operators need diagnostic information to process a claim. This information is communicated to senders using an International Classification of Diseases (ICD) code. CIM manuals are available from various publishers. Diagnostic information must be provided in ICD format when Medicaid billing is requested.
How does QuestDiagnostics work with the insurance?
QuestDiagnostics connects with the insurer first. The insurer will determine if the tests are incorporated by the respective plan. Any amount not covered is the patient responsibility to check.
How much should I pay?
The amount which will be the patients responsible for depends on the insurance amount.
